Hereby, the undersigned authorizes Roltrex Cargo Broker to sign and subscribe to all documents relating to this shipment. The above is a correct declaration and if the shipment is refused or returned for whatever reason, the undersigned agrees with all costs to be paid by him. The liability for warehouse and property is limited to $ 500 per vehicle and $ 40 per item, with a limit of $ 500. Insurance can be requested against additional payment at the Roltrex Cargo Broker office prior to export. The undersigned also agrees with all the terms and conditions of Roltrex Cargo Broker, as well as the Bill of Lading of the carrier.
